Fill in the applicable fields. Required information can be customized according to your company's PTW procedure.
Types of Work Permits
Different hazardous activities may require different permit controls. Select the permit type that matches the activity and always follow the applicable site PTW procedure.
🔥 Hot Work Permit
Used for activities capable of producing flames, sparks, heat or other ignition sources. Typical controls include fire prevention, suitable extinguishing equipment, gas testing where applicable and fire watch arrangements.
🕳️ Confined Space Permit
Used for controlled entry into qualifying confined spaces. Atmospheric testing, isolation, attendant arrangements, communication and rescue planning are important considerations.
🏗️ Work at Height Permit
Used where work involves a significant fall hazard. Controls may include suitable access, fall protection, inspection, barricading and rescue arrangements.
⛏️ Excavation Permit
Used to control excavation and ground-disturbance activities. Underground services, ground stability, access, edge protection and spoil placement should be assessed.
⚡ Electrical Work Permit
Used to control electrical work according to the site's electrical safety system. Isolation, verification, competent persons and electrical boundaries may be required.
🏗️ Lifting / Crane Permit
Used to support controlled lifting activities. Planning, equipment suitability, load information, exclusion zones and competent lifting personnel should be addressed.
What Is a Work Permit?
A Work Permit, commonly known as a Permit to Work (PTW), is a formal control document used to authorize specified work under defined conditions.
A properly managed permit-to-work process helps ensure that hazards are identified, necessary precautions are established, responsible persons are identified and work is authorized before an activity begins.
A permit should not be treated as a replacement for risk assessment, job safety analysis, method statements, isolation procedures or competent supervision. Instead, it forms part of a wider safety management system.
Why Is a Permit to Work Important?
Hazardous work can involve multiple people, departments, contractors, energy sources and simultaneous activities. A PTW process provides a structured way to communicate the conditions under which work may proceed.
- Defines the work scope and location.
- Identifies relevant hazards.
- Documents required control measures.
- Clarifies responsibilities.
- Records permit validity.
- Supports isolation and energy-control processes.
- Provides a formal authorization record.
- Supports communication between operations, maintenance and HSE personnel.
Typical Permit to Work Process
1. Identify the Work
Define exactly what work will be performed, where it will occur, who will perform it and what equipment or systems are involved.
2. Assess the Hazards
Review the hazards associated with the task and surrounding activities. The permit should work together with the site's risk assessment or JSA.
3. Establish Controls
Determine the necessary controls, including isolation, barricading, PPE, access control, fire prevention, atmospheric monitoring and emergency arrangements where applicable.
4. Authorize the Work
The responsible authorized persons review the conditions and approve the work according to the site's PTW procedure.
5. Monitor the Work
Work should remain within the permit conditions. If conditions change, the activity should be reassessed and the permit suspended, revised or reissued according to site procedure.
6. Close the Permit
Once work is complete, the responsible personnel should confirm that the area and equipment are left in an acceptable condition and close the permit according to the applicable process.
Work Permit vs Risk Assessment
A risk assessment identifies hazards and evaluates risks before selecting appropriate controls. A Work Permit is an authorization mechanism that establishes the conditions under which specified work can proceed.
The two systems complement each other and should not be considered interchangeable.
Important Permit Controls
Depending on the activity, a permit may need to address energy isolation, electrical isolation, gas testing, fire prevention, fall protection, confined-space controls, excavation precautions, lifting controls, environmental conditions and emergency arrangements.

Should a Work Permit replace a JSA or risk assessment?
No. A Work Permit should normally complement, rather than replace, the applicable risk assessment, JSA/JHA, method statement and isolation procedures.
This online Work Permit Generator provides a general-purpose editable template for planning and documentation. It is not a legal approval, engineering certification or substitute for a company's approved Permit to Work system, risk assessment, JSA/JHA, method statement, isolation/LOTO procedure, competent-person review or applicable laws and regulations.
Before using a generated permit for actual work, review and adapt it to the requirements of the specific workplace, project, jurisdiction and responsible authority.
